A: Client Applications for our service dogs are available for download on our "Client Application" page. You will need either Microsoft Word, which is part of Microsoft Office, or Adobe Screen Reader in order to view and print our client application.

Q: Does the $25.00 nonrefundable application fee guarantee that I will receive a Service Dog from NHAD?

A: No, the $25.00 nonrefundable application fee does not guarantee placement of one of our service dogs with you. This fee is to help cover our expenses incurred with processing applications.

Q: What age range does NHAD place their Service Dogs with?

A: We have placed service dogs with every age from Toddlers through Adults. We are beginning to focus on children though, because unlike NHAD, many organizations won't place a Service Dog with a child younger then Junior High/Middle School age. We have found that younger disabled children can benefit from having a Service Dog, just as much as an older disabled child or adult would.
